Application for Research with Human Subjects

All human subjects research projects must be submitted on the most current version of the application available below. It is important to check this website for updated versions, as OPHS will no longer accept outdated applications.

1. TO BEGIN: download the Application and open it with your word processor. To fill out the application, type your responses directly into the document. Be sure to answer ALL questions, even if you need to indicate that a question is non-applicable. Questions left unanswered will prolong the review process. 2. DATA ANALYSIS APPLICATION: If the research involves only the analysis of data that has already been collected (either for research purposes or non-research purposes) OR if the research involves only the analysis of data that has been collected and will continue to be collected in the future for non-research purposes (e.g. test scores, course evaluations, etc.) the researcher may use the Data Analysis Only Application.


3. ATTACHMENTS: These will be filled out only by those researchers for whom they are relevant. For example, Attachment A must be filled out by any researcher whose research is funded, but researchers who do not have funding are not required to fill out Attachment A. In addition to being listed below, links to the necessary attachments are provided throughout the application.

4. SUBMIT YOUR APPLICATION: Submit ONE copy of your application to OPHS. You can mail your application, fax it, or drop it off in person. You may also email it as an attachment provided that either the signature page has been physically signed and scanned into a PDF file or you have 'signed' the document using a digital image of your physical signature.
