Changing Your Name On Your UO Records

Students: Visit the Office of the Registrar and fill out the University of Oregon Change of Name form. If you’ve applied for a degree and wish to have your new name appear on your diploma, be sure to check the box provided on the form.

International Students: If you are not a US citizen, the name on your UO records must match the name on your passport. If you have questions about this, contact the Office of International Programs at (541)346-3206 or visit the OIP office, 330 Oregon Hall.

Student Employees, Faculty, Staff: If you have ever been or are currently employed by the University, your UO records must match the name on your Social Security card. If you officially change your legal name, take your updated social security card to the Payroll Office on the first floor of Oregon Hall to process your name change.
