APPLICATION PROCEDURES --
STEPS TO A SUCCESSFUL APPLICATION:

All of our forms and documents are in pdf format. Download Adobe Reader. It's free.

  1. Your application is due by January 1. You will be notified no later than mid-February.
  2. You must complete the online Graduate Application for the Graduate School, and pay a $50 non-refundable filing fee.
  3. You must send the following information to the Department of Human Physiology Office at the address listed below.

Please compile and send the following materials as one complete application package.
    1. Athletic Training Program Application Checklist
    2. Transcripts of all your prior academic work (unofficial transcripts may be sent with application package, providing official transcripts will follow).
    3. Graduate Record Examination (GRE) scores recorded on Athletic Training Program Application, with a copy of your exam score printout when possible.
    4. Scores from the Test Of English as a Foreign Language (TOEFL), if required.
    5. Letter of Introduction
In roughly one page please introduce yourself to us, let us know why you are interested in studying and working with us, what your clinical interests are, and what your primary goals are regarding your future education and career.
    6. Work Sample
Please answer the following questions about the article provided. Please ensure your response is thoughtfully written using a clear and concise scientific writing style. Answers should be 1-2 pages (double spaced, 12-pt Times New Roman or equivalent). We are interested in both your critical thinking skills, and your ability to communicate in writing. For tips on scientific writing please visit the Journal of Athletic Training’s web site.

      Reading:
Heitz N, Eisenman P, Beck C, and Walker J. Hormonal changes throughout the menstrual cycle and increased anterior cruciate ligament laxity in females. Journal of Athletic Training. 1999;43:144-149.

      Questions:
      1. What do the results of the study suggest, and how might their conclusion be related to the sex bias in ACL injury rate?
      2. What alterations could be made in the methodology to strengthen these results, or how could future studies be designed to help clarify these findings?
    7. A minimum of 3 references are required to complete your application. Be sure that at least 2 references address your academic performance and potential for success in graduate school, and at least 2 references describe your clinical athletic training experience and abilities (a letter from a direct supervisor who can quantify your athletic training experience is recommended). Your evaluator may address BOTH your academic performance AND your athletic training experience, if appropriate. Letters must be included in the completed application packet, paper-clipped to the completed Reference Form and sealed in an envelope signed along the seal.
Now that you have assembled all the appropriate materials (a test in itself), please submit the completed application package to:
