2008-2009 International Student Insurance

Because of the high cost of health care in the United States, University of Oregon international students and their accompanying dependents are required to have health and accident insurance which meets the University of Oregon requirements. (UO) (OAR 571-04-015)

The University offers a plan that is designed to meet the needs of UO international students. The coverage can be paid by the term or by the year. The plan provides coverage for treatment at the UO Health Center as well as treatment anywhere in the U.S.

International students will be automatically billed for the student insurance premium on their tuition bill. Students do not need to complete an enrollment form unless they wish to insure family members. Enrollment forms for dependents are available at the Student Health Insurance Office in the University Health Center.

Students may use alternate insurance as long as that insurance meets university requirements. See insurance plan requirements under Health Insurance Waiver below.

Eligibility- International undergraduate and graduate students registered for the current term and paying the mandatory fees including the Health Center fee will be automatically billed for the cost of the UO insurance plan.

Spouses and unmarried children under the age of 19 are eligible for coverage on the UO insurance plan. The eligible student must be covered by this insurance in order to insure family members.

Premium Payment - The health insurance premium cost will be billed on the student’s tuition bill. The premium is to be paid no later than 30 days after the beginning of the term to avoid a “Registration Hold” for the following term.

University Health Center Charges

When you receive medical services at the University Health Center, you will be asked how you want the charges billed. You need to request that charges be billed “Regular” billing.

The Health Insurance Office will bill eligible University Health Center charges directly to the insurance company for you. You do not need to request insurance statements or file claim forms.

If you pay CASH for services or ask for charges to be billed “CONFIDENTIAL”; those charges WILL NOT be billed to the insurance company for you. Contact the Health Insurance Office to make arrangements to bill these charges.

University Health Center charges not eligible for payment by the insurance company and your copayments amounts will be billed to your University account and must be paid by you.

Outside Medical Charges

It is the responsibility of the insured person to provide insurance information to medical providers when receiving medical care. Show the insurance ID card whenever receiving medical care. The insurance card lists the policy number and billing address.

If an insured pays cash for any medical services, you will need to provide the Health Insurance Office with receipts.

The insured must pay cash for all prescriptions. Outside pharmacies CANNOT bill the student health insurance for your prescriptions. You will need to provide the Health Insurance Office with the receipts from the pharmacy in order to be reimbursed.

All charges incurred with a medical provider are owed to that provider. It is the responsibility of the insured to make arrangement to pay the deductible and copayment to the medical provider who provided the medical care.
