Influenza Clinics for Students, Faculty/Staff- 2008
Students:
Dates: Monday-Friday, starting Monday, October 06, 2008
(No Flu Vaccination Clinics on November 27 & 28)
Time: 9-11:30AM & 1-4PM
Where: University Health Center at the Nurse Specialty Clinic or at your regularly scheduled appointment
Cost: $15 - cash or check (no credit cards) or can be billed to your student account
Faculty/Staff:
Who:
- UO faculty/staff, spouses/domestic partners and/or dependent children age 13 and older.
- UO retirees for whom Medicare is not the primary insurance provider: Proof of primary insurance must be provided.
- The University Health Center does not provide primary care to Medicare beneficiaries. The Health Center is unable to immunize anyone for whom Medicare is the primary carrier.
- Eligible dependents 13-17 years of age require parental consent for immunization and must be accompanied by UO faculty/staff member. Eligible dependents 18 years of age and older, if not accompanied by faculty/staff, must bring the employee's UO ID along with the Regence Blue Cross Blue Shield of Oregon PEBB insurance card.
Weekly Clinics: Each Wednesday, Thursday and Friday starting Wednesday, October 08, 2008 and continuing throughout fall term
(No Flu Vaccination Clinics on Wednesday, October 29, and Thursday and Friday, November 27 & 28)
Time: 8AM-9AM
Where: University Health Center at the Nurse Specialty Clinic (13th and Agate)
What to bring:
- UO ID card
- Regence Blue Cross Blue Shield of Oregon PEBB insurance card or cash/check
- If retired, proof of primary insurance (Blue Cross or other)
- Those individuals who present with a UO ID card and their Regence Blue Cross Blue Shield of Oregon PEBB insurance card will be eligible to receive the flu vaccine without charge
- No Regence Blue Cross Blue Shield of Oregon PEBB insurance - charge is $30, cash or check (no credit cards)
